Relate the prescribing record, discharge-summary EIA and AI-generated summary, testing whether later change or derivation affects reliance.
Make the conflict explicit
Use provenance, timing, authority and reconciliation evidence to establish Current Confidence and the material limitation.
Control downstream reliance
Place the IIP Under Review, use authorised clinical judgement and update or recall affected records and summaries.

What confidence is justified?

- Useful for the wider discharge plan, but not yet the confirmed medication position.
